Pay: $24.04 per hour

What You'll Be Doing:

• Under the guidance of the HAY Center Housing Campus Supervisor, the Housing Case Manager will work with youths ages 18 through 24 who are referred to the HAY Center Housing Continuum.

• The Housing Case Manager will support youth through the housing and leasing process at the HAY Center Foundation Apartments. This includes assisting youth with navigating the housing voucher application process, completing required applications, and attending housing briefings

• The Housing Case Manager will provide ongoing supportive case management services to youth residing at the HAY Center Foundation Apartments to promote housing stability and support a successful transition toward independent living.

• Completes housing assessments to determine housing readiness and provides case management/supportive services needed to ensure youth are prepared for the housing opportunities available.

• Assists youth through the housing process including application completion and voucher process.

• Assists the youth in completing and submitting documentation as needed to the local housing authorities throughout the housing process including the initial application, recertifications, move requests, income changes and family composition changes to ensure the youth remains in compliance with the housing program.

• Assist youth in the move-in process at the HAY Center’s Foundation Apartments

• Provides onsite case management to youth residing on the HAY Center’s Foundation Apartments

• Attends and participates in wraparound meetings with youth as needed.

• Completes required case documentation in a case management database system and maintains case files for assigned cases to ensure that all information regarding the youth's housing plan is current and updates are available to the HAY Center team.

• Works closely with internal programs with an emphasis on the In-care Program: ensuring youth have information regarding available housing programs and how to prepare for housing requirements.

• Attends and participates in all HAY Center and Housing Continuum events and workshops as needed

What We're Looking For:

• Bachelor’s degree Social Services, Psychology, Social Work, Sociology, Human Services, Criminal Justice or closely related field

• Computer Skills

• Knowledge in Microsoft Word, Microsoft Excel, Data Entry, Microsoft Outlook

• Knowledge of principles, practices, and techniques of case management and administration, case management documentation and records management

• Some experience working with/knowledge of public housing and housing subsidy

• Some experience working with/knowledge of transitional needs of youth aging out of foster care
