Bring your passion for youth advocacy, housing stability, and community impact to a role that truly changes lives. Openwork is seeking a Housing Readiness Case Manager to support young people ages 17.5–24 as they prepare for safe, stable housing and independent living.
This position offers meaningful, relationship-driven work within a supportive housing continuum designed to help youth successfully transition into adulthood.
As a Housing Readiness Case Manager, you will guide youth through the housing preparation and leasing process while providing consistent, strengths-based case management. Your work will help ensure young people are informed, supported, and equipped to secure housing and maintain stability as they move toward independence.
You will play a vital role in removing barriers, coordinating services, and helping youth build confidence and readiness for long-term housing success.
Conduct housing readiness assessments and provide individualized case management and supportive services
Assist youth with housing applications, voucher processes, and required housing briefings
Complete, collect, and submit documentation to local housing authorities, including recertifications
Support youth through the move-in process
Provide onsite case management services to youth in supportive housing
Participate in wraparound meetings and collaborate with internal programs as needed
Maintain accurate case files and complete required data entry
Participate in housing-related and community events as needed
Bachelor’s degree in Social Services, Psychology, Social Work, Sociology, Criminal Justice, or a closely related field
Proficiency in Microsoft Word, Excel, Outlook, and data entry systems
Knowledge of case management principles, documentation, and records management
Familiarity with public housing programs and housing subsidies
Experience working with youth aging out of foster care or navigating transitional housing
This role involves regular interaction with youth in a supportive housing setting and requires strong organizational skills, adaptability, and collaboration across teams.
Reliable transportation required
Valid driver’s license and current liability insurance
Pay: $22.46 per hour (weekly pay)
Schedule: Monday–Friday (some weekends as discussed during interview)
