Housing Case Manager – Supportive Housing Program
Location: Houston, TX (Gulf Freeway area)
Pay Rate: $26.00 per hour (Paid Weekly)

Openwork is partnering with a respected, community-focused organization to hire a dedicated Housing Case Manager for a dynamic and meaningful role within a supportive housing program.
This position blends hands-on housing navigation with individualized, strengths-based case management. You will play a critical role in helping young adults secure stable housing, develop life skills, and overcome barriers to long-term success.
The Housing Case Manager partners closely with youth as they navigate the housing process—from voucher applications and documentation to lease-up and move-in. Once housed, you will provide consistent, ongoing support to help clients maintain stability, achieve personal goals, and build independence.
This is a highly engaging, in-person role requiring strong relationship-building skills, organization, and a genuine commitment to youth empowerment.
Conduct housing readiness assessments and deliver individualized case management and supportive services
Guide youth through housing applications and voucher processes, including required forms and housing briefings
Assist with documentation submitted to local housing authorities, including:
Initial applications
Recertifications
Move requests
Income updates
Family composition changes
Support youth throughout the move-in process to ensure a smooth housing transition
Provide ongoing onsite case management within supportive housing settings
Participate in wraparound meetings to coordinate services and ensure holistic support
Maintain accurate and timely documentation in a case management database system
Collaborate with internal programs to educate youth on housing options and preparation strategies
Attend and support program events and workshops as needed
Bachelor’s degree in Social Services, Psychology, Social Work, Sociology, Human Services, Criminal Justice, or a closely related field.
Proficiency in Microsoft Word, Excel, Outlook, and data entry systems
Strong organizational, time management, and documentation skills
Ability to build trusting, supportive relationships with youth
Effective communication skills and the ability to collaborate across multidisciplinary teams
Knowledge of case management principles, documentation standards, and records management
Experience with public housing programs and housing subsidies
Experience working with youth aging out of foster care or navigating transitional housing
This position requires a fully open and flexible schedule, including availability to work weekends and late weekends as needed. Flexibility is essential to meet the needs of youth and program operations.
Regular, in-person engagement with youth in a supportive housing setting
Ability to exercise sound judgment and collaborate effectively across teams
Reliable transportation required
Valid driver’s license and current liability insurance required
